ADA Applications
If you are unable to use regular fixed-route bus service due to a disability, or if you are age 65 or older, you may be eligible for CATARIDE shared-ride service. To apply, please select the appropriate application below and follow the instructions provided. All applications must be completed and submitted for review before eligibility is determined.
ADA Paratransit Application (Disabilities)
For individuals whose disability prevents them from using regular bus service.

- Must be completed in full
- May require additional documentation
Senior Application (Age 65 and Over)
For individuals age 65+ who qualify for CATARIDE service.
- Simple application process
- Proof of age may be required
How to Submit
Completed applications can be submitted by:
- Mail – Follow instructions included in the application
- In Person – At the CATA office
- Fax or Email – If listed on the form
What Happens Next
Once your application is received:
- It will be reviewed by CATA staff
- Additional information may be requested
- You will be notified of your eligibility status
